People with dementia often want to continue to live at home rather than move to residential care, and there are physical, emotional, cognitive, social, and economic benefits to older people staying at home if they can, including preserving physical and mental health, independence, autonomy, and maintaining connections to family, friends, social groups, neighborhoods, and communities. Although family carers provide much of the care, paid, at-home care is often needed as an adjunct to maintain independence, health, and safety of the person with dementia, particularly as symptoms progress. Randomized controlled trials have shown home care contributes to reduced hospital admissions, delayed institutionalization, and improved quality of life

Service Provided during the home visit to - Person with dementia:

Assessment of the person with dementia – identify any clinical issues like infections, bedsores, hypertension, diabetes etc.
Assessment of the person’s cognitive abilities, behavioral changes, and progression of dementia.
Design a care plan and explain it to the family carers.
Provide care for any clinical issues like bed sores, catheters, wound management etc.

Assessment of the home environment to see if it is dementia enabling and friendly.
Provision of cognitive stimulation therapies and validation therapies as required.
Facilitation of referrals for any specialist services, physiotherapy, and rehabilitation.

It can be over whelming, caring for a loved one with Alzheimer’s or any type of Dementia. The family care givers often suffer from symptoms like anxiety, depression, sleeplessness, exhaustion, social withdrawal, and various health problems.

The home care team will counsel the family on:

Caregiver stress management. Understanding the person with dementia.
Management of the challenging behaviors.
Identifying the distressed behavior as a response to unmet needs.
Environment modification and adaptation.
Communication and engagement of the person with dementia in meaningful activities.
Financial and legal planning and advanced medical directives for the person with dementia.
